A cognitive view of 100 Tonne takeoff weight error, close shave for AirFrance

A 100 Tonne error in take off performance. We continue to believe that an engineering control will prevent a future incident. We need to ensure that flight crew perform NORMAL actions/procedures consistently and with accuracy. Human factors should be the point in focus.

ET302, Pilots on a wing and a prayer: mindFly

ET 302 took off from Addis Ababa, the capital of Ethiopia on 10th March 2019 and 06 minutes into the flight met with an accident. The aircraft was the Boeing’s latest addition to the B-737 series, the B-737 8Max. It soon became a world wide ban, thus grounding the MAX fleet. I will not critique on the crew action or inaction because their behaviour will need to be…

Erroneous takeoff performance: El Al flight takes off with a 40Tonne error@mindFly

The El Al B-789 managed to get airborne safely despite the 40 Tonne error in weight while calculating the takeoff performance.
I have written a paper on how and why humans commit such grave errors and ways and means to prevent them.
