Not MCAS but lack of trust grounds B737Max worldwide:mindFly

The notorious MAS which was suspected by many to be the cause of the Lion Air accident in 2018 end may not be the cause in the Ethiopian Airlines ET302 accident. The reason why the big regulators like UK, France, Singapore, Australia and many other are imposing a ban on the B-737 Max to even enter their airspace points to a lack of trust in FAA and Boeing and the actions taken by them in the past. Read my blog “A question of safety or ethics” http://mindfly.blog/2019/01/10/a-question-of-safety-or-ethicsboeing-737. There was a legal case in the USA in early 2018 where the…

The ABC of behaviour: mindFly

The reaction of crew to a particular situation can be predicted ,but only in theory. Practically, there are a number of variables that result in a behaviour and these are in a constant state of flux. Threats are defined as “events or errors that occur beyond the influence of the flight crew, increase operational complexity, and which must be managed to maintain the margins of safety”. During typical flight operations, flight crews have to manage various contextual complexities. Such complexities would include, for example, dealing with adverse meteorological conditions, airports surrounded by high mountains, congested airspace, aircraft malfunctions, errors committed…

What is fear; passenger evacuation? mindFly

Passenger evacuation from an aircraft after an incident is the last resort where the crew decides that the safest option in the interest of minimizing harm to the passengers and crew is to leave the aircraft as soon possible before a ravaging fire engulfs the aircraft or any other eventuality. Even under these high risk situations, passengers have been observed to take their time to collect their personal belongings from the overhead stowage before they exit the aircraft. There is no sense of urgency in their actions or behaviour. The choice of the door to exit is also based on…

38 accidents like AirAsia,1000 million USD spent on a training which will fade away in no time: mindFly

On average, it takes more than two months before a new behavior becomes automatic — 66 days to be exact. There are no shortcuts!

Plausible scientific theory of MH370 location: mindFly

Video giving detailed explanation Just after midnight on March 8, 2014, Malaysia Airlines’ flight MH370, a Boeing 777-200ER with 239 persons on board, took off from Kuala Lumpur on its way to Beijing. One hour later, it disappeared from air traffic controllers’ radar screens and was reported missing. One year later, the Malaysian authorities published an interim report. The Australian Transport Safety Bureau (ATSB) and the American company Ocean Infinity then used the report to guide their search campaigns in 2017 and 2018, but all three proved to be fruitless. In July 2018, the Malaysian authorities published the final report…
