Pilots don’t want DGCA to paint all incidents with the same brush

The tenets of Just culture mandated by ICAO and adopted by DGCA differentiate between an honest mistake and a deliberate one/gross negligence. The DGCA enforcement procedure manual follows the same and categorises administrative action into counselling, warning and in case of wilful neglect, suspension of pilot license. However, the investigations do not determine systemic faults but rather bring about individual faults. Instead of root causes, they stop at pilot error.
